Acute Care: Acute Care Forum Action Planning Tool
Help your Acute Care Forum check their agenda or action plans are up to date and locally relevant and that the membership and any "sub-group" arrangement of your forum is appropriate to deliver the action plans.
The action plan is based on the Capable Acute Care Forum in accordance with the National CSIP guidance.
Additional actions can be added locally and the tool provides a summary of all the action points using a "traffic light" chart.
