This is a modification of the Activity and Outcomes database to suit the information needs of a Crisis Resolution Home Treatments Team.
Whilst intended to address one Trusts specific and immediate needs for managing Home Treatment referrals and gate keeping, its scope can be extended to include referrals, activity and discharges across all functional teams, and the database application, or its data model, could be adopted by any Trust.
