National Pathfinder Sites
Stoke-on-Trent PCT was awarded Pathfinder status for the West Midlands region. Led by local commissioners Stoke used its Pathfinder funding to develop a ‘Health Minds Network' providing quicker access to psychological therapies and integrating services provided by both statutory and third sector agencies. The ten sites across the country developed service models for different sections of the community and provide evidence of best practice. A guide to commissioning for the whole community and a series of Positive practice guides have been produced. click here
